Additional Named Applicants |
You can one or more additional applicants. Add and delete rows, and order the applicants as necessary. You can then add information for the additional applicants. |
Locations |
Location information is required for all applications. This can be just for the applicant’s home address or place of work, or it can be for multiple premises. Add and delete rows as necessary, using the plus and trash icons respectively. Optionally, you can specify the first location as the applicant mailing address. |
Class Codes and Rating |
A class code is required for all applications. This can be for one location or more than one, including ones located in different states. You can enter a different class code for each location you have listed. When you have more than one location, in the Loc # field enter the number that corresponds with the location you are describing. (For example, if you are entering class codes and rating information for the third listed location, enter 3.) To select the state class code for the location, first select the state. In the class field, enter a few digits of the code or a few letters of its description. For states that do not use or have class codes available, enter the data yourself. You may also need to enter a governing state and classification. To view geographic location click Map in the navigation bar. |
